CONSENT FORM Ikwezi Community Development Centre (Aligned to accredited training provider requirements) I, the undersigned learner, hereby confirm that I have read, understood, and agreed to the following terms and conditions in accordance with applicable training provider policies, procedures, and learner obligations . 1.
