CONSENT FORM Ikwezi Community Development Centre (Aligned to accredited training provider requirements) I, the undersigned learner, hereby confirm that I have read, understood, and agreed to the following terms and conditions in accordance with applicable training provider policies, procedures, and learner obligations . 1. Registration Fee & Refund Policy 1.1 The registration fee is an administrative and non-credit-bearing fee charged to cover application processing, learner registration, system capturing, compliance administration, and onboarding costs. 1.2 A request for a refund of the registration fee must be submitted in writing within three (3) working days from the date of payment, provided that learner registration has not yet been finalised. 1.3 No refund of the registration fee will be granted where: The learner has already been formally admitted, enrolled, or registered on institutional systems, as administrative and compliance costs would have been incurred...
