(Aligned to accredited training provider requirements)
I, the undersigned learner, hereby confirm that I have read, understood, and agreed to the following terms and conditions in accordance with applicable training provider policies, procedures, and learner obligations.
1. Registration Fee & Refund Policy
1.1 The registration fee is an administrative and non-credit-bearing fee charged to cover application processing, learner registration, system capturing, compliance administration, and onboarding costs.
1.2 A request for a refund of the registration fee must be submitted in writing within three (3) working days from the date of payment, provided that learner registration has not yet been finalised.
1.3 No refund of the registration fee will be granted where:
- The learner has already been formally admitted, enrolled, or registered on institutional systems, as administrative and compliance costs would have been incurred. Or;
- The request is submitted after the three (3) working day period.
2. Attendance and Punctuality
2.1 Learners are required to attend all scheduled contact sessions, practicals, and assessments punctually and consistently.
2.2 A minimum attendance rate of 90% is required to remain compliant with programme and assessment requirements.
3. Absenteeism and Learner Conduct
3.1 All absences and late arrivals must be formally communicated to the training provider.
3.2 In cases of illness or emergency, a valid medical certificate or supporting documentation must be submitted.
3.3 Absence for a continuous period exceeding one (1) month without approved communication may result in:
- Learner withdrawal or termination of enrolment; and/or
- A requirement to re-register, subject to institutional approval.
4. Assessments and Examinations
4.1 Learners must present themselves on time for all formative and summative assessments.
4.2 Once assessment or examination dates have been scheduled or confirmed, failure to attend may result in the learner forfeiting the assessment opportunity, subject to institutional assessment rules.
4.3 Re-assessment opportunities, where applicable, remain subject to provider policies and procedures.
5. Fees and Payments
5.1 All applicable tuition and programme-related fees must be paid in full and upfront by the registration date, and no later than the 7th of each month, where applicable.
5.2 Non-payment or late payment may result in the learner being temporarily suspended from training activities until the account is brought up to date.
6. Certification and Graduation Costs
6.1 Certification, statements of results, graduation, and related costs are excluded from the course fees and will be charged separately, where applicable.
Declaration
I acknowledge that I understand my rights and responsibilities as a learner and agree to abide by the terms and conditions set out above.
This consent form is implemented in alignment with the Consumer Protection Act 68 of 2008 and applicable training provider regulations.
